Some of this will sound familiar.
None of it shows up as a fault. That is what makes it expensive.
A lead arrives and nobody picks it up. By the time anyone notices, they have already spoken to somebody else.
The deal closes. Finance invoices a different number to the one that was agreed. Someone finds it at month end.
Sales signs the account. The customer team hears about it a week later, from the customer.
Two systems, two answers, one meeting. Nobody can say which figure is right, so nobody trusts either.
Nothing failed. No alert went off. The work simply stopped, and nothing said so.
